I, the undersigned person, have read a description of this pet adoption event and/or have been briefed on the activity and understand that this activity (“Event”) may involve hazards to me and my real or personal property. However, I am willing, and hereby agree, to take and assume all risk of such hazards in order to participate in the Event.

I hereby represent that I am at least eighteen years old and have the authority and capacity to enter into this Release and Waiver ("Release"). I understand and represent that I am not acting as an agent, authorized representative or employee of Subaru of America, Inc. ("SOA") and not entitled to any benefits which may accrue thereto.

I understand that SOA cannot guarantee the behavior or health of any animal during the Event, including possible exposure to infectious diseases, including certain zoonotic diseases that are transmissible from animals to humans. In consideration of being allowed to participate in the Event, I hereby release and hold harmless SOA, its parent and affiliates, and each of their directors, officers, employees, and agents (the "SOA Releasees") from any and all claims that may arise from or relate to my participation in the Event, including, but not limited to, negligence on the part of any of them.

In no event shall I hold the SOA Releasees liable for any loss or expense, including third party complaints or litigation, resulting from the actions or inaction of the SOA Releasees. I shall not attempt to hold the SOA Releasees responsible for any damages which any animal during the Event may cause to any person or property.

I hereby grant SOA and Subaru Corporation ("SBR") a non-exclusive right to use photographs and video taken during the above event for use in promoting SOA's and SBR's business and charitable event promotion. Use may include but is not limited to website, social media, annual reports, brochures, and electronic and print media.

I have read this Release and Waiver and freely and voluntarily, without any inducement, intend my signature to be a complete and unconditional release of the SOA Releasees from all liability.

For parents/guardians of Minors: I hereby represent that I am the parent and/or legal guardian of the minor child(ren) listed below and have the authority to enter into this Release and Waiver on the Child's behalf. I agree to supervise the minor children listed below while they are participating in the Event.
